Program

Liberty Community Services, Inc. operates a 16 bed transitional living program, a 7 bed permanent supportive living program, a 25 bed independent scattered site program, a 22 bed Safe Haven, a collaborative 26 bed intensive service scattered site program, and housing access and referral resource program.

To ensure quality housing and support programs for the under served designed to reduce the impact of poverty and homelessness on each individual and our community.

Background

Liberty Community Services, Inc. (formerly CARP, Inc.) has provided services to persons living with HIV/AIDS since 1987. In addition to its HIV/AIDS service programs, Liberty Community Services offers a variety of supportive housing and service options focused to meet the changing face of poverty, including programs specifically targeted to those living with mental illness and/or addiction.

Population Served

Liberty Community Services offers a variety of programs targeted at a multiplicity of special needs populations, including: Homeless or at risk of homelessness adults (all programs); Adults living with HIV/AIDS; Substance addicted; and Mentally ill.

Services Provided

Liberty Community Services offers a wide range of programs for homeless or at risk of becoming homeless adults living with HIV/AIDS. These programs include the Transitional Living Program, the Supportive Living Program, the Independent Living Program, Safe Haven and The Open Door Alliance (a PILOTS Initiative). Services provided by these programs include case management, counseling, addiction and recovery support, independent living skills development and access to rental assistance.
