Background
Chrysalis Center has been providing services to persons in need since 1975. The organization has been providing services to persons living with HIV/AIDS since 1990. The program was designed to provide flexible, comprehensive and wrap-around services tailored to meet the needs of individuals (and their families) diagnosed with HIV/AIDS.
Population Served
Chrysalis Center serves adults and families living with HIV/AIDS who are homeless or at risk of homelessness, as well as individuals and families with multiple disabilities/illnesses and those difficult to engage. Chrysalis Center serves individuals and families who reside in Hartford, West Hartford, Bloomfield, East Hartford and Wethersfield.
Services Provided
Chrysalis Center provides a wide array of community based services to individuals and families in need of housing supports and case management services. Chrysalis Center provides assistance in obtaining and maintaining safe, decent, affordable housing; linkage to rental assistance when needed and available; assistance in securing entitlements; and assistance in accessing and coordinating medical, substance abuse and mental health care. Chrysalis Center also provides individual, family and group counseling, support groups, health education, medication monitoring support.
Additional Features
Chrysalis Center is a host agency for a national demonstration project, Hudson View Commons, Soromundi Commons in Hartford. These projects bring together housing and support services to individuals who are living with mental illness, HIV/AIDS and/or struggling with substance addictions, many of whom have been homeless or at risk of homelessness. Chrysalis Center’s case management services are accredited by CARF, the Commission on Accreditation of Rehabilitation Facilities.
